Making a brighter future path for our students
Transition Coaches work in collaboration with your student’s teacher and supporting the student’s successful transition needs. Whether it be finding a job prior to graduation, career exploration, setting up workplace skills experiences, working with the re-integration team, ensuring a smooth transition back to a less restrictive environment, transitioning to adult day programs, or post-graduation follow up.

Direction for the future
Almost all HS students graduating from the Austin Centers, with parent collaboration, were placed in post-secondary programming:
- Military
- Community College (over 10 college students in 2 years)
- Day Programs (through DDD)
- Vocational Rehabilitation supported Job Coaching
- 30+ students assisted in obtaining part-time jobs by assisting with resume building, job interview skills, and workplace skills earned on campus.
- Facilitated and toured over 20+ day program facilities around the valley with students, parents, and district representatives.
- Annual Transition EXPO in which 20+ post-grad programs come and educate parents about what they have to offer. This is open to the public.
Austin Centers transition program also supports families by hosting:
- Guardianship workshops at no cost
- Transition Expo (over 20 vendors in attendance)
- Community College tours
- DDD contracted Day Program tours for post-graduation placement
- Attending students annual IEP meeting to support the transition from school to adulthood programming
